For a child, the Lifespan Waiver is not the route: DDDS lists it for ages 18 and up, and its manual reserves waiver-funded specialized medical equipment for people 21 and over. A child's equipment request goes through Medicaid EPSDT on medical necessity, which is a hard fit for play equipment. If your child's therapist believes a specific item is medically necessary, ask the Medicaid plan how it would be classified. Approval is decided case by case in your child's individual service plan, usually with an OT or PT recommendation attached, and most waivers pay only after regular Medicaid or private insurance has said no. No state approves play equipment automatically.
